Freelance Inquiry Form

Thank you for your interest in my art! By filling out this form you are applying for a commission piece, and we will come to terms based on your answers below.

Before we get started, please read below:

➥ Pricing

The size, form, type of art and shipping cost are all filtered into your total and will be agreed upon before I begin. Built in checkin points on a biweekly/monthly plan will be filtered in between start and finish dates to ensure that the final payments is received by the shipping date. Tracking will be provided. Payments Methods are due in INVOICES ONLY and will be sent directly to your email.

Fees are broken down into 3 equal payments:

1. Deposit: This is due once I contact you after completing the form.

2. Layout Checkin: I will show you the sketch pre-paint. This is your time to ask any questions or make changes.

3. Final Approval/Shipping Day: I will show you the completed piece, if you are satisfied I will ship it out same day.

➥ Deadline

Please take into consideration that project completion ranges between 4-12 weeks depending on the combination of size, form & type. If it is needed at an earlier or rush time frame please inform me once I contact you. Rush fees can vary. Your deadline should not be the exact date needed due to shipping time frames.

➥ Copyright and Usage Rights

I have full ownership of the artwork once completed. I have the right to use the artwork for promotional purposes. You are 100% allowed to share your commissioned artwork on social media. If you do choose to tag me, @kozycreativestudios

➥ Revisions Policy

You have up to 2 weeks from our initial contact to make revised changes. You are responsible for responding in a timely manner in all cases. To make any changes please contact me via email or text, I will get back with you as soon as possible. After I send the sketched draft to confirm that I move on to the next stage, no revisions can be made as I move onto the next layer(s). If it is deemed necessary, based on how far I have gotten, revisions can be made for an extra fee.

➥ Cancellation Policy

If you want to cancel and have only paid the deposit you can receive it back in full. If you have made your second payment there are no refunds.

Any information you have shared on and off the form are confidential and will not be shared beyond the art work.
